Modernize Home and Community-Based Services
Modernize Home and Community-Based Services: Comprehensive Data Infrastructure and Service Delivery Transformation
This initiative will explore opportunities to modernize Home and Community-Based Services (HCBS) to improve cost efficiency and better align these services with the needs and preferences of older adults and people with disabilities. It will use evidence-based instruments and leverage enhanced federal funding mechanisms to strengthen the overall long-term services and supports (LTSS) infrastructure. The goal is to create a more responsive, sustainable, and person-centered service delivery model.
The modernization will:
Evaluate the current HCBS assessment tool and explore implementation of a different tool (or suite of tools) with established reliability and validity, in order to better identify client needs, improve care planning, and provide for service refinement.
Ensure maximization of Medicaid Administrative Claiming with the goal of increasing federal matching funds for programs that currently rely on unmatched state funding.
Invest in cross-agency information technology infrastructure that allows for comprehensive, person-centered care coordination and service delivery.
